First Visit

Welcome to the world of outstanding oral health care at Upstate Oral Surgery & Dental Implants. Our goal is to help patients live happier, healthier lives by providing a wide range of oral surgery services to patients of all ages. The first visit is a great time for Dr. Curtis to get to know you and learn about your condition. Then, Dr. Curtis will explain your diagnosis and treatment options. In some cases, surgery can be performed the same day as a consultation, but this could change depending on a complex medical history or a more thorough treatment plan that requires additional appointments.

On your visit, we recommend that you arrive at our Watertown, NY, office a few minutes early to check-in and complete some paperwork. A parent or legal guardian must be present for patients under 18 years of age during the consultation.

Please bring the following items with you to your consultation:

  • Your surgical referral slip and any X-rays (if possible).
  • A list of your current medications.
  • Completed necessary dental or medical insurance forms.

Anesthesia Options

Upstate Oral Surgery & Dental Implants offers both local and general anesthesia to help you feel comfortable and at ease during your treatment. During your consultation, Dr. Curtis will discuss your anesthesia options and will decide which one will be used during your procedure. This is also a good time to bring up any anxiety you are experiencing about treatment because we want to answer all of your questions and make you feel comfortable.

We have detailed pre- and post-operative procedure instructions for you to read very closely and follow prior to your upcoming procedure. If you have any questions regarding these instructions, please feel free to contact our office and speak with our helpful and knowledgeable staff. We are here to address your concerns and ensure you have a smooth recovery.

Cancellation Policy

Cancellations of all appointments are allowed up to 48 hours. The following fees will apply if the patient does not give the office a 48-hour notice or does not show up for the scheduled appointment: $150.00 for local extractions, $500.00 for sedation procedures, and $1,000.00 for implant procedures.

Our office is an enrolled CareCredit provider. CareCredit is a credit card that you can use to cover out-of-pocket medical expenses that are not covered by insurance. It takes only minutes to apply and once approved, you can use your CareCredit credit card at any enrolled provider nationwide. You determine the short or long-term repayment option that fits your lifestyle, and CareCredit is there to help you afford the care you need. You can use your credit card for dental care, at hospitals and surgical centers, and even to care for your pet. Once approved, you will not need to reapply. There is no activation fee.

Payment is due at the time of service unless other arrangements have been made in advance. Please keep in mind that you are fully responsible for all fees charged by Upstate Oral Surgery & Dental Implants regardless of your insurance coverage.

Our staff works with you to try to maximize your insurance reimbursement for covered procedures and treatments. If you have any questions, our team is here to answer them. We ask that you bring your insurance information with you to your consultation appointment so that we can expedite reimbursement.
