The first visit is a great time for Dr. Curtis to get to know you and learn about your condition. Then, Dr. Curtis will explain your diagnosis and treatment options. In some cases, surgery can be performed the same day as a consultation, but this could change depending on a complex medical history or a more thorough treatment plan that requires additional appointments.
What to Bring
On your visit, we recommend that you arrive at our Watertown, NY, office a few minutes early to check-in and complete some paperwork. A parent or legal guardian must be present for patients under 18 years of age during the consultation.
Please bring the following items with you to your consultation:
- Your surgical referral slip and any X-rays (if possible).
- A list of your current medications.
- Completed necessary dental or medical insurance forms.

Cancellation Policy
Cancellations of all appointments are allowed up to 48 hours. The following fees will apply if the patient does not give the office a 48-hour notice or does not show up for the scheduled appointment: $150.00 for local extractions, $500.00 for sedation procedures, and $1,000.00 for implant procedures.
